2010 SA SCRAPBOOK CONVENTION: DELEGATE BOOKING FORM
- Complete the required fields to book your place at the event.
- Only one delegate per registration.
- Please advise payment plan amounts and dates to be paid in the 'Additional
Information' box.
- Delegate bookings will only be confirmed once deposit or payment has been received.

TERMS AND CONDITIONS:
1. The fee of R2495.00 includes all classes, class kits, teas, lunches and sponsor give-aways.
2. The Early Bird Offer for the 2010 SA Scrapbook Convention is as follows:
- Book and pay by the end of October 2009 and receive an AMM Tote-ally cool desktop tote OR
- Book and pay by the end of December 2009 and receive a 10% discount, ie R2495.00 less
R250.00 – R2245.00 OR
- Book and pay by the end of January 2010 and receive a 5% discount, ie R2495.00 less
R125.00 -  R2370.00. 
3. To qualify for the Early Bird Offer, full payment must be received by respective dates as stated above. Should you cancel at any time prior to the event, R350.00 will be deducted as well as the 25% cancellation fee.
4. Registration will take place on:
Johannesburg - Emperors Palace: Friday 10 or Saturday 11 September 2010 
Cape Town - Spier Estate: Friday 17 September or Saturday 18 September 2010
5. A non-refundable deposit of R300.00 is required when making a booking.
6. The fee of R2495.00 can be paid in instalments with the final payment made by 3 September 2010.
7. All bank deposit and EFT payments must be faxed to 086 552 1074 or 011 425 6215.
8. A cancellation fee of 25% of the full amount will be charged should you cancel your booking.
9. No refunds will be given whatsoever should you cancel 30 days prior to the event. Should you cancel 30 days or less prior to the event, you will forfeit your payment, but the kits will be kept and can be collected after the event. If you are unable to attend a class, this class will be forfeited and no sharing of days between delegates.
10. Full payment must be received by 3 September unless prior arrangements have been made.
